Do not use the Educational Activity Charitable Contribution Request form if
you are requesting financial support for a Third Party Meeting - such as an educational
grant for:
- Sponsoring a meeting for health care professionals, such as a conference
or grand grounds.
- Sponsoring a meal or reception at a Third Party Meeting.
- Providing a scholarship for health care professionals in training
to attend a Third Party Meeting.

Scholarships for Health Care Professionals in Training to Attend a Third Party Meeting
Do not use the Educational Activity Charitable Contribution Request Form if
you are requesting a scholarship to help pay for health care professional in
training (e.g., student, resident, or fellow) to attend a Third Party Meeting.

and request forms for a scholarship to help pay the cost of a health care professional
in training to attend a Third Party Meeting.
Scholarships are not available for health care professionals who have completed their training. We support their participation by sponsoring Third Party Meetings, which reduces overall costs, including registration fees.

MSD's charitable contribution request forms are formatted with MicroSoft's form tool. To complete the request form:
- Move to each field that needs to be completed by using the Tab button or
a moving your curser to the next field.
- If the field is a box, then just click on the appropriate box to place an
"x" in the box.
- If the field is a gray text area (it is empty white space in a printed version
of the form), simply start typing in the requested information.
- If you need to unlock the form to add information that does not have a box
or text field: (1) select "View" on the top menu bar, (2) select "Toolbars,"
(3) select the "Forms" toolbar. Once the toolbar is added, click on the icon
that looks like a padlock so it is no longer highlighted. You can then type
anywhere on this document. If you click on the padlock again, you may lose
the information you entered in a box or text field - so avoid unlocking the document until you have completed all of the form fields and then do
not re-lcok it.
In order to assure readability, MSD prefers that all request forms be typed rather than handwritten.
Submit the form according to the directions on the form. To avoid any delays
in reviewing your request, please be sure to complete the request form and provide
all of the requested information and documents.

Educational Activity Charitable
Contribution Criteria
In addition to the criteria that apply to all Customer-Based Charitable Contributions,
an educational activity for which as charitable contribution is requested must
also satisfy the following additional criteria:
- Promotes public or health care professional education or awareness about spine trauma, deformity, or degeneration or associated neurological
disease states and medical conditions, including identification, diagnosis, and treatment.
- Is educational or scientific and not intended to promote MSD products or
a health care professional's private practice.

Business Conduct Standard Brochure
MSD's Compliance Office has a "Charitable Contributions Program" brochure
that provides answers to common questions people have.
To order the brochure, you need access to MySpineTools.com.
If you do not have access, you can ask your local MSD sales representative for
the brochure or send an email to MSDCharitableContributions@medtronic.com
and ask for the brochure.
If you have access to MySpineTools.com, the brochure can be ordered on MySpineTools.com
by selecting "Resources" from the top navigation bar and then, from the Overview

What types of educational events and activities will
MSD support through charitable contributions?
Activities and events that promote public or health care professional education
and awareness about disease states and medical conditions that MSD Products
are intended to address. For example, public television shows about spinal
deformity, trauma, or degeneration. Another example, would be general education
brochures or pamphlets about spinal conditions treated by MSD Products.
Can educational charitable contributions be used for co-marketing promotion
with our Customers?
No. Even if the Customer is a tax-exempt charitable organization, MSD's education
charitable contributions cannot be used to help finance co-marketing promotions such as referral dinners, conferences, and newsletters for patients, primary care physicians and/or other potential referral sources for the Customer.
Are scholarships for Third Party Meetings handled as an education charitable
contribution?
No. Even though the scholarship recipient may be a charitable organization,
all Third Party Meeting scholarships are handled under Business Conduct Standard
